			<edb:english>Insulin preserves fast-twitch muscle mass during early STZ-induced diabetes in rats.</edb:english>

			<edb:english>PURPOSE: Skeletal muscle atrophy is a critical complication of type 1 diabetes mellitus (T1DM), primarily caused by insulin deficiency. While physical activity is known to improve metabolic health, its effectiveness in preventing diabetes-induced muscle atrophy, particularly in the absence of insulin, remains unclear. This study examined the effects of voluntary exercise on muscle atrophy in an early phase streptozotocin (STZ)-induced diabetic rat model. METHODS: STZ was administered to male Wistar rats and SPORTS rats, which maintain high activity levels even after diabetes onset. Blood glucose levels, skeletal muscle mass, and muscle atrophy-related gene expression (Atrogin-1 and MuRF1) were measured, as well as locomotor activity. The effects of insulin, the sodium-glucose cotransporter (SGLT) inhibitor phlorizin, and leucine supplementation were also examined. RESULTS: Extensor digitorum longus (EDL) muscle mass decreased significantly within seven days of diabetes onset, accompanied by early expression of atrophy-related genes. Insulin administration prevented the decrease in EDL muscle mass, whereas phlorizin did not, despite improving hyperglycemia. SPORTS rats maintained high voluntary locomotor activity after diabetes onset but failed to prevent muscle atrophy. Metabolomic analysis indicated altered branched-chain amino acid (BCAA) metabolism in diabetic muscle tissue. Leucine supplementation partially attenuated muscle atrophy in STZ-treated diabetic rats. CONCLUSIONS: These findings suggest that insulin deficiency, rather than decreased physical activity, is a major contributor to early muscle atrophy in this model. Voluntary exercise alone is insufficient to prevent STZ-induced muscle atrophy in the absence of insulin. Leucine supplementation may offer a potential therapeutic strategy for muscle preservation.</edb:english>

			<edb:english>Metabolic dysfunction-associated steatohepatitis (MASH) is characterized by hepatocellular injury, macrophage activation, and severe fibrosis, and often progresses to liver cirrhosis and hepatocellular carcinoma. Excessive accumulation of visceral fat exacerbates hepatic inflammation and fibrosis independently of fatty liver, but the underlying molecular mechanisms have remained unclear. We here identify MFG-E8 (milk fat globule-EGF8) as a secreted protein that is overexpressed in adipose tissue of obese mice and contributes to such exacerbation. MFG-E8 deficiency in MASH model (STAM-MASH) mice was associated with reduced hepatic expression of inflammation- and fibrosis-related genes without attenuation of steatosis. Conversely, MFG-E8 supplementation in MFG-E8 knockout mice intensified hepatic inflammation and promoted the formation of hepatic crownlike structures. Coculture of macrophages with apoptotic hepatocytes induced expression of inflammatory cytokine genes, and this effect was enhanced by the presence of exogenous MFG-E8 in the culture medium. Our findings suggest that adipose tissue-derived MFG-E8 infiltrates the liver and promotes macrophage-hepatocyte interaction, thereby contributing to hepatic inflammation and fibrosis in MASH.</edb:english>

			<edb:english>The skeletal muscle plays a key role in thermogenesis and energy homeostasis in endotherms. Therefore, reduced skeletal muscle mass and function are closely associated with health disorders such as obesity and hypothermia. In humans, inactivity and nutritional deficiencies can lead to skeletal muscle atrophy. However, hibernating mammals, which can greatly suppress their metabolic rate, can maintain significant skeletal muscle mass even during prolonged periods of inactivity and nutritional restriction. This review focuses on how skeletal muscle contributes to maintaining body temperature as the organ that consumes the most energy, while also contributing to whole-organism homeostasis through its high metabolic flexibility in a self-sacrificing manner. Particularly, we reconceptualized muscle atrophy associated with the thermoregulatory process in terms of inter-organ metabolic interaction, proposing that sarcopenia is an integral component of systemic energy metabolism regulation. By deepening our understanding of the functional metabolic flexibility of skeletal muscle and its regulatory mechanisms, we can redefine sarcopenia as an adaptive response that contributes to maintaining metabolic homeostasis. This perspective could provide new insights into the pathophysiology of sarcopenia and metabolic disorders, and inform the development of more effective prevention and treatment strategies.</edb:english>

			<edb:english>Endotherms increase the rate of metabolism in metabolic organs as one strategy to cope with a decline in the temperature of the external environment. However, an additional major contributor to maintenance of body temperature in a cold environment is contraction-based thermogenesis in skeletal muscle. Here, we show that impairment of hind limb muscle contraction by cast immobilization induced a loss of function of skeletal muscle and activated brown adipose tissue (BAT) thermogenesis as a compensatory mechanism. BAT utilizes free branched-chain amino acids (BCAAs) derived from skeletal muscle as an energy substrate for thermogenesis, and interleukin-6 released by skeletal muscle stimulates BCAAs production in muscle for support of BAT thermogenesis. Additionally, this thermoregulatory system between BAT and skeletal muscle may also play an important role in response to cold temperatures or acute stress. Our findings suggest that BAT and skeletal muscle cooperate to maintain body temperature in endotherms.</edb:english>

			<edb:english>Prediction of Sarcopenia Onset in Type 2 Diabetes Using Urinary Titin Levels: A Japanese Prospective Cohort Study.</edb:english>

			<edb:english>OBJECTIVE: The predictive power of urinary titin for incident sarcopenia was studied in Japanese individuals with type 2 diabetes. RESEARCH DESIGN AND METHODS: Baseline urinary titin levels were measured, and sarcopenia was evaluated annually using the Asian Working Group for Sarcopenia (AWGS) 2019 criteria. Kaplan-Meier curves, Cox models, and time-dependent receiver operating characteristic analyses were used to assess incident sarcopenia. RESULTS: Among 444 participants (median follow-up, 1,078 days), 41 developed sarcopenia. The high titin tertile was associated with an elevated sarcopenia risk (log-rank P = 0.04). Cox models associated titin with sarcopenia (adjusted hazard ratio per SD 1.37, 95% CI 1.05-1.77, P = 0.019) and low muscle strength. Risk estimates were consistent across subgroups, including those aged ≥70 years, men, individuals with BMI &lt;25 kg/m2, HbA1c ≥7%, and estimated glomerular filtration rate ≥60 mL/min/1.73 m2 (P for interaction &gt; 0.05). CONCLUSIONS: Elevated urinary titin levels predict sarcopenia and low muscle strength in individuals with type 2 diabetes, supporting its use as a noninvasive biomarker.</edb:english>

			<edb:english>Limb-girdle muscular dystrophy type R1 (LGMDR1, formerly LGMD2A) is a genetic disorder caused by mutations in CAPN3 and is characterized by progressive proximal limb muscle weakness. The CAPN3 gene product, calpain-3/CAPN3/p94, is a member of the intracellular cysteine protease superfamily predominantly expressed in the skeletal muscle. LGMDR1 pathogenesis has been investigated separately using mouse models: CAPN3:C129S [knock-in (KI)] mice, which express a proteolytically inactive variant, and CAPN3 knockout (KO) mice. These studies propose that CAPN3 bears both proteolytic activity-dependent and -independent functions and that the loss of either or both affects phenotypes. Here, we report a side-by-side, long-term analysis of KI and KO mice to comprehensively understand the LGMDR1 pathology in terms of CAPN3 function. Their physiques were comparable to those of wild-type animals, but age-dependent LGMDR1 symptoms were observed by histochemical analysis, with more severe symptoms observed in KO mice. Quantitative muscle proteomics and gene ontology analyses revealed more diverse changes in the KO mice than in the KI mice. Of the associated terms, &quot;metabolic process&quot; was the most affected across the genotype and age groups. Metabolomic analysis suggested that the skeletal muscles of these mice had an imbalance in the branched-chain amino acid catabolic pathway. Furthermore, a reduction in lipids and glycogen was observed in the liver of KO mice, suggesting that a systemic energy deficit occurs during CAPN3 deficiency. Altogether, our results suggest that muscular dysfunction in LGMDR1 models is associated with compromised systemic energy balance and that the extent of perturbation is implicated in disease severity.</edb:english>

We evaluated changes in adipogenic and thermogenic gene expression, mitochondrial activity, and fatty acid uptake following 5-AzaC treatment in 3T3-L1-derived beige adipocytes. Our findings indicate that 5-AzaC does not significantly affect adipocyte differentiation but enhances thermogenic gene expression, including Ucp1 and Cox7a1, and increases mitochondrial function and fatty acid uptake. These results suggest that DNA demethylation modulates beige adipocyte function and may have implications for metabolic disorders such as obesity and type 2 diabetes.</edb:english>

			<edb:english>Daily Energy and Protein Intake Requirements to Prevent Body Weight Loss in Elderly Residents of Nursing Homes.</edb:english>

			<edb:english>Background Maintaining body weight is crucial for preserving the health of older adults. This study aimed to identify the daily energy and protein intake levels necessary to prevent body weight loss exceeding 1 kg over a six-month period among nursing home residents. Methodology A retrospective analysis was conducted on 101 residents aged ≥75 years. Data on body weight changes and dietary intake over a continuous six-month period were reviewed. The association between dietary intake and body weight loss greater than 1 kg was evaluated in 78 female residents. Results Among the 101 residents (23 males and 78 females; median age: 91 years (interquartile range (IQR) = 86-94); median body mass index (BMI): 21.3 kg/m² (IQR = 18.7-23.7)), 41% experienced a body weight loss of more than 1 kg over six months, despite receiving a median energy intake of 24.7 kcal/kg/day (IQR = 19.6-29.2) and a protein intake of 0.98 g/kg/day (IQR = 0.81-1.20). Multivariate logistic regression analysis adjusted for age and BMI in female residents revealed that an energy intake exceeding 30 kcal/kg/day was significantly associated with a reduced risk of body weight loss (odds ratio (OR) = 0.17, 95% confidence interval (CI) 0.04-0.68, p = 0.013), as was protein intake exceeding 1.2 g/kg/day (OR = 0.12, 95% CI = 0.03-0.49, p = 0.003). Conclusions To prevent body weight loss in elderly female nursing home residents, a daily intake of at least 30 kcal/kg/day of energy and 1.2 g/kg/day of protein is recommended. These findings highlight the importance of proactive nutritional management in long-term care facilities to support better health outcomes in this vulnerable population.</edb:english>

			<edb:english>The Spontaneously Running Tokushima Shikoku (SPORTS) rat is characterized by hyperactive behavior but is also a potential model for studies of the role of the gut-pancreas axis in the regulation of insulin secretion. We here explored the role of ghrelin, a hormone associated with appetite regulation, in insulin dynamics within the context of streptozotocin (STZ) resistance in SPORTS rats. Comprehensive analyses-including histological examinations, gene expression profiling, and assessment of metabolic parameters-revealed that SPORTS rats are resistant to STZ-induced pancreatic injury and that, in addition to low circulating ghrelin levels, they manifest increased circulating levels of active glucagon-like peptide-1 (GLP-1) and upregulated expression of Pdx1 in the pancreas. Ghrelin administration attenuated the STZ resistance of these rats, with suppression of GLP-1 and downregulation of Pdx1 expression being implicated in this effect. Our results highlight the complex interplay among ghrelin, GLP-1, and insulin dynamics, and they suggest potential new therapeutic targets for diabetes.</edb:english>

			<edb:english>Perioperative malnutrition is common in patients with gastroenterological cancer and contributes to postoperative skeletal muscle atrophy, which adversely affects their prognosis. Early assessment of skeletal muscle atrophy is crucial for improving postoperative outcomes. This study aimed to evaluate the efficacy of urinary titin as a biomarker for skeletal muscle atrophy. A prospective observational study was conducted, and a total of 34 gastroenterological cancer patients were included. Urinary titin levels were measured using ELISA at admission, postoperative days (POD) 1, 7, and 14, and at 6 months after surgery. Surgical procedure, operative time, cancer stage, postoperative complications, hospital stay, and preoperative and postoperative body composition were evaluated, along with nutritional status and grip strength from admission to 6 months after surgery. Changes in urinary titin levels were measured at the same time points as described above. Preoperatively, the mean urinary titin level was 5.03 pmol/mg Cr, slightly higher than in healthy subjects. Urinary titin peaked at 33.71 (24.30-66.58) pmol/mg/dL Cr on POD1 and was associated with serum free branched-chain amino acid concentrations. Urinary titin on POD1 was significantly correlated with a decrease in skeletal muscle mass (rs -0.361, p = 0.036) and body cell mass (rs -0.361, p = 0.038) at 6 months postoperatively. The grip strength at 6 months postoperatively tended to decrease (rs -0.342, p = 0.052). BMI and serum LDH at admission were associated with urinary titin on POD1 but were not correlated with skeletal muscle loss at 6 months, suggesting that urinary titin on POD1 is an independent biomarker of skeletal muscle atrophy. These data indicate that urinary titin on POD1 can predict long-term skeletal muscle atrophy.</edb:english>

			<edb:english>Objective;In order to maintain a patient&apos;s adequate nutritional status, it is important to accurately assess energy expenditure and to provide nutritional administration commensurate with that expenditure. In this study, we have assessed the resting energy expenditures (REE) in patients with esophageal and head and neck cancer, and association between REE, metabolites, nutritional intake, and body composition. Methods;Patients included in the study had to be diagnosed with esophageal and head and neck cancer, and hospitalized and receiving chemotherapy. REE was measured using indirect calorimetry with a mask and calculated using 3 different equations. Result;Patients with esophageal and head and neck cancer (67.1±8.5 years old, n=52, all patients were male) were enrolled. The average of REE measured by indirect calorimetry were 22.1kcal/kg/day. REE was positively correlated with muscle mass, body cell mass, and lean body mass. REE was higher in group with oral intake of more than 72%. Blood levels of lactate, pyruvate, and amino acids were higher in cancer patients with esophageal and head and neck cancer compared to healthy subjects. Conclusion:REE measured by indirect calorimetry correlated with the estimated BEE. There was an association between energy expenditure and muscle mass and nutritional intake. J. Med. 			<edb:english>Skeletal muscle atrophy impairs quality of life and increases the risk of disease, but current methods for assessment of muscle mass have several limitations. We here investigated the urinary concentration of a fragment of the muscle protein titin as a potential biomarker for the early detection of skeletal muscle atrophy. Four mouse models with different atrophy pathways were studied: those of cardiotoxin-induced acute muscle injury, cast-induced muscle immobilization, lipopolysaccharide-induced sepsis, and streptozotocin-induced diabetes. In all four models, urinary titin levels increased early, concurrent with or preceding upregulation of the atrophy-related genes for atrogin-1 and MuRF-1. The increase in the urinary titin concentration was thus associated with initial muscle damage and the onset of proteolysis, rather than with late-stage muscle wasting. Our findings suggest that urinary titin is a promising biomarker for detection of the onset of skeletal muscle catabolism and prediction of the subsequent development of atrophy in different catabolic states. Noninvasive measurement of urinary titin may therefore allow the earlier detection of skeletal muscle proteolysis compared with conventional techniques.</edb:english>

			<edb:english>Impact of Olfactory Change on Postoperative Body Weight Loss in Patients with Gastric Cancer after Gastrectomy.</edb:english>

		<edb:article.summary>
			<edb:english>Patients undergoing gastrectomy for gastric cancer may experience alterations in olfaction, yet the association between olfactory changes and postoperative weight loss remains uncertain. This study aimed to elucidate the relationship between olfactory changes and postoperative weight loss in patients with gastric cancer. Patients who underwent radical gastrectomy for gastric cancer between February 2022 and August 2022 were included in the study. Those experiencing a higher Visual Analog Scale (VAS) score postoperatively compared to preoperatively were deemed to have undergone olfactory changes. Postoperative weight loss was determined using the 75th percentile as a cutoff value, designating patients surpassing this threshold as experiencing significant weight loss. Multivariate logistic regression analysis was employed to identify risk factors for postoperative weight loss, with statistical significance set at p &lt; 0.05. Out of 58 patients, 10 (17.2%) exhibited olfactory changes. The rate of postoperative weight loss at one month was markedly higher in the group with olfactory changes compared to those without (9.6% versus 6.2%, respectively; p = 0.002). In addition, the group experiencing olfactory changes demonstrated significantly lower energy intake compared to the group without such changes (1050 kcal versus 1250 kcal, respectively; p = 0.029). Logistic regression analysis revealed olfactory changes as an independent risk factor for significant weight loss at one month postoperatively (odds ratio: 7.64, 95% confidence interval: 1.09-71.85, p = 0.048). In conclusion, olfactory changes emerged as an independent risk factor for postoperative weight loss at one month in patients with gastric cancer following gastrectomy.</edb:english>

			<edb:english>Partially hydrolyzed guar gum (PHGG) is a soluble dietary fiber that is effective for defecation control. It influences the gut microbiota, by which it is metabolized to yield short-chain fatty acids (SCFAs), and it was also recently shown to protect against influenza infection in humans. We here investigated the effects of PHGG in a mouse model of influenza H1N1 virus infection. Eight-week-old C57BL/6 mice were fed normal chow with or without PHGG (500 mg/kg per day) for 4 weeks, infected with H1N1 at 10 weeks of age, and analyzed at 12 weeks of age. Administration of PHGG attenuated the decline in body weight induced by H1N1 infection without affecting food intake. It also ameliorated intestinal atrophy and increased the production of SCFAs including acetic acid, propionic acid, and butyric acid in the cecum, thereby preventing the inhibitory effect of H1N1 infection on SCFA production. The H1N1-induced increases in the serum concentrations of inflammatory cytokines including interferon-γ and interleukin-6 and anti-inflammatory cytokine such as interleukin-10 were all inhibited by PHGG intake. In addition, PHGG administration attenuated inflammatory gene expression in the lung and promoted both natural killer cell activity and regulatory T-cell differentiation in the spleen. Our findings suggest that the consumption of PHGG may improve the gut environment and thereby limit the inflammatory response to H1N1 infection. They may thus provide the basis for novel dietary intervention strategies to suppress the excessive inflammation associated with virus infection.</edb:english>

			<edb:english>OBJECTIVE: Dysgeusia is a serious problem in patients with diabetes because it often leads to overeating, which is associated with disease progression. This study aimed to investigate the association between taste sensitivity, eating habits, and the oral environment. SUBJECTS AND METHODS: In this cross-sectional study of 75 subjects with diabetes, gustatory function was assessed using the whole-mouth method, and lingual taste receptor gene expression was measured by real-time PCR. Food intake was evaluated using a food frequency questionnaire based on food groups. The oral environment was assessed using xerostomia and periodontal comprehensive examination. RESULTS: In total, 45.3%, 28.0%, and 18.7% of subjects showed lower umami taste sensitivity, low sweet taste sensitivity, and low salt taste sensitivity, respectively. Lower umami sensitivity correlated with lower estimated glomerular filtration rate and higher energy-source food intake. Subjects with diabetes with higher plaque control record showed significantly higher T1R3 gene expression than those with lower plaque control record. CONCLUSION: Reduced umami taste sensitivity is associated with decreased renal function and high energy food intake in diabetes. Subjects with diabetes with higher plaque control record showed significantly higher T1R3 gene expression, suggesting that the oral environment affects taste gene expression. J. Med. Here, we focused on lingual taste receptor gene expression in diabetes and elucidated the relationship between taste receptor gene expression and renal function. Forty-seven patients with diabetes and 10 healthy subjects (control group) were enrolled. Lingual foliate papillae were scraped and the derived cDNA was quantified by real-time polymerase chain reaction. Dysgeusia was assessed using SALSAVE?. All statistical analyses were performed using JMP? software 13. The expression of T1R1 and T1R2 was significantly upregulated in type 2 diabetes patients as compared with that in healthy subjects (P&lt;0.01) but did not change in type 1 diabetes patients. T1R3 expression positively correlated and Scnn1 expression negatively correlated with estimated glomerular filtration rate, suggesting that altered taste receptor gene expression could reflect impaired renal function. Thus, alterations in T1R3 and Scnn1 expression in diabetes correlated with renal function. 			<edb:english>Fish oil-derived long-chain monounsaturated fatty acids (LCMUFAs) with a carbon chain length longer than 18 units ameliorate cardiovascular risk in mice. In this study, we investigated whether LCMUFAs could improve endothelial functions in mice and humans. In a double-blind, randomized, placebo-controlled, parallel-group, multi-center study, healthy subjects were randomly assigned to either an LCMUFA oil (saury oil) or a control oil (olive and tuna oils) group. Sixty subjects were enrolled and administrated each oil for 4 weeks. For the animal study, ApoE-/- mice were fed a Western diet supplemented with 3% of either gadoleic acid (C20:1) or cetoleic acid (C22:1) for 12 weeks. Participants from the LCMUFA group showed improvements in endothelial function and a lower trimethylamine-N-oxide level, which is a predictor of coronary artery disease. C20:1 and C22:1 oils significantly improved atherosclerotic lesions and plasma levels of several inflammatory cytokines, including IL-6 and TNF-α. These beneficial effects were consistent with an improvement in the gut microbiota environment, as evident from the decreased ratio of Firmicutes and/ or Bacteroidetes, increase in the abundance of Akkermansia, and upregulation of short-chain fatty acid (SCFA)-induced glucagon-like peptide-1 (GLP-1) expression and serum GLP-1 level. These data suggest that LCMUFAs alter the microbiota environment that stimulate the production of SCFAs, resulting in the induction of GLP-1 secretion. Fish oil-derived long-chain monounsaturated fatty acids might thus help to protect against cardiovascular disease.</edb:english>

			<edb:english>Assessment of insulin resistance in the skeletal muscle of mice using positron emission tomography/computed tomography imaging.</edb:english>

			<edb:english>Measuring glucose uptake in the skeletal muscle in vivo is an effective method to determine glucose metabolism abnormalities as the skeletal muscle is the principal tissue responsible for glucose disposal and is a major site of peripheral insulin resistance. In this study, we investigated the pathological glucose metabolism dynamics of the skeletal muscle of C57BL/6J mice in a noninvasive and time-sequential manner using positron emission tomography/computed tomography (PET/CT), an imaging technique that uses radioactive substances to visualize and measure metabolic processes in the body, with [18F]-fluoro-2-deoxy-D-glucose (FDG). FDG-PET/CT imaging revealed that insulin administration and exercise load significantly increased FDG accumulation in the skeletal muscle of C57BL/6J mice. FDG accumulation was lower in the skeletal muscle of 14-week-old db/db diabetic model mice exhibiting remarkable insulin resistance compared to that of 7-week-old db/db mice. Based on the continuous observation of FDG accumulation over time in diet-induced obese (DIO) mice, FDG accumulation significantly decreased in 17-week-old mice after the acquisition of insulin resistance. Although insulin-induced glucose uptake in the skeletal muscle was markedly attenuated in 20-week-old DIO mice that had already developed insulin resistance, exercise load effectively increased FDG uptake in the skeletal muscle. Thus, we successfully confirmed that glucose uptake accompanied by insulin administration and exercise load increased in the skeletal muscle using PET-CT. FDG-PET/CT might be an effective tool that could noninvasively capture the chronological changes of metabolic abnormalities in the skeletal muscle of mice.</edb:english>

			<edb:japanese>Metabolic syndrome is a pathological condition with hyperglycemia, dyslipidemia, or hypertension resulting in cardiovascular and cerebrovascular disease. Factors that affect visceral fat accumulation and weight gain include not only physiological factors such as heredity, constitution, and age, but also behavioral factors and environmental factors. Eating behavior and eating environment are critical for the prevention and treatment of metabolic syndrome. In this report, we will discuss about recent topics of nutrition in metabolic syndrome, especially association with macronutrients such as carbohydrate, fatty acids and protein. In recent years, several publications have determined the benefit of low carbohydrate diet on obesity and diabetes, and the American Diabetes Association has also recommended low carbohydrate diet, Mediterranean diet, and a diet mainly consistent with vegetables and plants. There have been many reports that the effect of carbohydrate restriction has not only the weight loss effect but also reduces risk factors such as cardiovascular disease and cerebrovascular disease. Fanelli et al . recently have shown that half of US adults with diabetes have less than the recommended daily intake of protein, which may lead to poor dietary quality. Diabetic patients with low protein intake had significantly lower dietary nutrient density and 12．5% higher carbohydrate intake, whereas diabetic patients with daily protein intake meeting the recommended amount have shown that the overall quality of the diet was high and almost met the recommended daily amount of vegetables, whole grains, dairy products and added sugars. Therefore, in addition to the prevention of metabolic syndrome, protein is important for treatment of metabolic syndrome and higher QOL. Nutritional research needs further development to deal with metabolic syndrome.</edb:japanese>

			<edb:japanese>Skeletal muscle atrophy is a serious problem in critically ill patients. After intensive care unit (ICU)admission, muscle atrophy starts within 3 days and progressively worsens thereafter. We have previously reported that upper and lower limb muscle mass of critically ill patients decreased by 13%‐21% within 7 days of admission. Muscle atrophy in the ICU is associated with long-term functional impairment and weakness in ICU survivors. Although monitoring of muscle mass is important, there have been no reliable biomarkers associated with muscle atrophy in critically ill patients. In this study, we have focused on urinary titin, which is the biggest giant sarcomere protein and functions as a spring for muscle extension and viscoelasticity. We have clarified that urinary titin level reflected muscle breakdown correlated with limb muscle atrophy in non-surgical critically ill patients. The cumulative urinary titin level associated with the incidence of ICU-AW and ICU mortality. Our study indicates urinary titin can be the noble biomarker to evaluate the catabolism and muscle atrophy in critically ill patients.</edb:japanese>
