Sandblasted spherical steel balls with diameters of 10-20 mm were measured using ARCTICA and a three-dimensional measurement device (FN503). The radius of each steel ball was calculated from the measured value and the difference between the measured and nominal values was obtained. Upper/lower dental arch casts were measured and processed for occlusal contact image creation using both devices. The graphical images of the occlusal contacts at the intercuspal position obtained from both systems and an occlusal analysis device (BE-1) were compared with the occlusal contact area. Excellent correlation was observed between the measurement results of ARCTICA and FN503 (p<0.001, R: 0.99). The occlusal contact areas were 186.0 mm (ARCTICA), 192.8 mm (FN503), and 196.1 mm (BE-1). Materials and Methods: In 10 adult volunteers who had healthy normal dentition, frontal and lateral border movement was measured during unilateral mastication and free mastication, using an ultrasound digital mandibular measuring system (ARCUS digma2). Additionally, precise impressions of the upper and lower dental arch were taken, and dental casts were made. These casts were measured using a CAD/CAM scanning system (ARCTICA). For the fabrication of crown restorations, the functionally generated path technique (FGP technique) was used on the monitor for the lower first and second molar. The movement of the opposite teeth on the occlusal surface during frontal and lateral border movement, during unilateral mastication, and during free mastication was considered the functional occlusal surface. The data of the functional occlusal surface generated by frontal/lateral mandibular translations and that of each of the masticatory functional occlusal surfaces were superimposed using three-dimensional data evaluation software (GOM). The difference between these surfaces was evaluated to determine the interference area, maximum interference difference, and average interference difference.
Results: Interference was present for all functional occlusal surfaces created by mastication as well as those determined by frontal/lateral mandibular translations. The average interference values, in order of free masticatory movement, habitual masticatory side, and non-habitual masticatory side, were as follows. Interference area: 167.5 ± 20.8 mm2, 121.9 ± 28.5 mm2, 144.6 ± 28.0 mm2; maximum interference distance: 345.0 ± 43.1 m, 189.0 ± 39.9 m, 309.0 ± 46.8 m; average interference distance: 130.0 ± 15.7 m, 64.0 ± 10.6 m, 130.0 ± 21.9 m. Statistically significant differences were found for the maximum interference distance and average interference distance (both p < 0.05).
Conclusions: The functional occlusal surfaces for each form of mastication demonstrated interference with the functional surface of frontal/lateral mandibular translations. Seventy-nine care workers used the assessment sheet to evaluate oral conditions in 188 institutionalized older adults. Forty-five care workers and three dental professionals then assessed oral conditions in 45 institutionalized older adults using the test-retest method before and after dental professional guidance. The patient was able to manage this affliction through rapid-repetition jaw opening and closing. Tomography of the joint showed no irregular morphology, but intraoral examination revealed an occlusal interference at the mandibular left third molar during leftwards excursion. For this patient, alteration of lateral guidance using a palatal plate attached to the maxillary left canine precluded this intermittent open lock, but at 22 years of age, the open lock recurred and could not be relieved by the patient, who was unable to assume an occlusal position. Because conservative treatment was ineffective, a pumping manipulation technique was applied to reduce the open lock, after which the patient has maintained good jaw function. MRI taken before and after repositioning indicated that abrupt reduction of a displaced articular disk was the cause of the open lock, and that this articular disk was restored to its proper position during the manipulation. Most TMJ open locks occur as anterior dislocation, where the mandibular head becomes trapped anterior to the articular eminences, causing excessive opening and difficulty closing. Our clinical findings from this patient indicate that open lock can occur through abrupt reduction of a displaced articular disk, particularly in patients with chronic internal derangement of the TMJ. "徳島大学病院における顎関節症患者の特徴に関する調査-男女比,年齢構成と受診期間-","A hospital-based survey of TMD patients at Tokushima University Hospital","竹内 久裕, 石川 輝明, 松本 文博, 田中 栄二, 中野 雅德","Hisahiro Takeuchi, Teruaki Ishikawa, Fumihiro Matsumoto, Eiji Tanaka, Masanori Nakano","null","目的:徳島大学病院で使用している顎機能レポートシステム記録から受診患者の特徴と経過を把握し,さらに臨床サンプルにおけるバイアスの影響について検討を行うことを目的とした. 対象および方法:対象は徳島大学病院を顎関節症または関連疾患で受診し,顎機能レポートシステムに記録があり,なおかつ選択基準を満たした解析対象群1,083例(男女比1:2.4)と歯科診療部門をほぼ同期間に受診した一般初診患者群24,588例(男女比1:1.4)である.解析項目は年齢構成,男女比,受診期間などである. 結果および考察:解析対象群の年齢構成は15∼24歳と55∼59歳にピークのある二峰性を示した.一般初診患者群との比較では,解析対象群の15∼34歳が多い傾向を示す一方,60歳以上では少ない傾向を示し,男女比も有意に異なっていた(χ2検定:p<0.001).年齢分布については,一般住民の年齢構成(総務省「平成17年国勢調査」)に対し一般初診患者群,解析対象群とも青壮年層で少ない傾向を示し,大学病院受診患者というサンプリングバイアスが見かけ上の二峰性を生じさせていた可能性が考えられた.経過としては90日以内で約80%,180日以内で約90%が診療終了となる一方,通院1年以上の長期症例が約5%存在した. 結論:大学病院における顎関節症患者の特徴および経過について明らかにするとともに,サンプリングバイアスの影響について検討を行った.","Objective: The purpose of this research was to clarify the characteristics of TMD patients at Tokushima University Hospital and to investigate the effects of clinical sampling bias. Materials and Methods: Recorded data of selected TMD patients (1,083 cases, average age 39.2±19.4, male : female=1 : 2.4) from the TMD report system in the digital chart system of the hospital and new patients of Tokushima University Hospital Dental Division (24,588 cases, average age 43.0±23.5, male : female=1 : 1.4) were analyzed in terms of the age distribution, the male to female ratio and the treatment period. Results: The male to female ratio of both groups was significantly different (chi-square test: p<0.001). The age distribution of TMD patients was bimodal. Peak age groups were 15-24 and 55-59 years old. In comparison with the new patients group, there were many TMD patients among the 15-34 year-old group, but few in the 60-84 year-old group. On the contrary, the age distributions were different from those of the general population in Tokushima prefecture (Population Census 2005, Ministry of Internal Affairs and Communications), particularly among the middle aged. This comparison indicated that the bimodal age distribution of TMD patients might reflect a sampling bias of the clinic-based sample. For example, the limited office hours for outpatient care of the university hospital might result in a socio-economic bias for working-age patients and cause an apparent bimodal distribution. Regarding treatment period, about 80% of patients completed regular outpatient treatment within 90 days and about 90% of patients completed it within 180 days. On the other hand, there were 52 cases (4.8% of 1,083 cases) who received outpatient treatment for more than 1 year. On the other hand,the questionnaire to the participating persoonnel was conducted in order to evalaote this program as the regional contrtbution. As a result,this program is useful to elderly people and the understanding of institution personnel is also deepened this practice. As mentioned above,the result of having is suitable for the educational goal ""to have a consciousness of becoming the dental professional"". This issue is particularly relevant to dental professionals because they are exposed to various allergens. The objective of the present study is to conduct an allergy survey among dental professionals and clarify the actual prevalence and other parameters of allergies.Methods: An anonymous questionnaire was distributed to employees and dental students working at the University of Tokushima Dental Hospital (currently, the Department of Dentistry, Tokushima University Hospital).Results: Valid responses were obtained from 294 individuals (145 males and 149 females). The average age of the respondents was 34.0 years. Of the respondents, 61.2% had experienced some subjective symptoms associated with allergy. The most common allergic symptom was allergic rhinitis (40.1%), and 20.4% of the respondents perceived themselves to be allergic to medical gloves.Conclusion: The study results indicated that the prevalence of subjective allergic symptoms was higher among dental professionals than among the general public or other healthcare professionals. Even the 3-D image reconstructed from spiral CT data has some distortion, typically along to the table feed direction, very few studies were reported about accuracy of reconstructed 3-D image size. The purpose of this research was to evaluate the distortion on 3-D image reconstructed with spiral CT (Somatom Plas4, Siemens Co, Germany) quantitatively. We used three imaging targets, eight ceramic balls placed at corners of cubic frame, a calibration cube that has the CT value nearly equal to cortex bone of human dried mandibular head. We evaluated relation between a threshold level of CT image processing and reconstructed 3-D image size with a custom-made computer program. CT data of ceramic balls clari>ed changes of the reconstructed image, -0.215mm for table feed direction and less than ±0.006mm for the other directions. The calibration cube was used for the arrangement of optimal scanning condition (reconstruction mode, collimation, reconstitution interval) and evaluation of reconstructed image size. The distortion of the image was +0.207mm for table feed direction and less than +0.029mm for the other directions. Changes of dried skull TMJ image reconstructed with the optimal scanning condition were -1.463mm for lateral direction of the condylar head, -0.956mm for back and forth direction and -0.580mm for table feed direction. Results of this research suggested that the level of the 3-D image distortion was mostly affected with the scanning condition. And the size of 3-D image was susceptible to the threshold level of CT image processing. The right side upper first premolar movement of an adult volunteer without any signs or symptoms of periodontal disease was studied. A small occlusal table mounted on a force transducer was clipped between the upper and lower dentition. Occlusal force was applied to the buccal cusp, palatal cusp or central fossa of the first premolar and also to the right side canine aspect by clenching this occlusal table. The right side first premolar movement relative to the left side first premolar was measured by means of a highresolution six degrees of freedom motion detector. The three-dimensional shape of the first premolar was measured with following methods. A 3-D digitizer with a touch sensor probe was used to measure the coronal part of a cast stone model of the tooth. Computerized tomography data were used to obtain the root shape of the tooth. These movement and configuration data were combined to perform computer graphics analysis of the three-dimensional tooth movements. The range and direction of tooth movement were varied by altering the position and amplitude of occlusal force loading. Since the magnitude of the tooth movements was relatively small, we adapted the screw axis (helical axis) concept to emphasize tooth movement on the computer graphics representations.","The purpose of this research was to evaluate tooth movement in six degrees of freedom under occlusal force loading. The right side upper first premolar movement of an adult volunteer without any signs or symptoms of periodontal disease was studied. A small occlusal table mounted on a force transducer was clipped between the upper and lower dentition. Occlusal force was applied to the buccal cusp, palatal cusp or central fossa of the first premolar and also to the right side canine aspect by clenching this occlusal table. The right side first premolar movement relative to the left side first premolar was measured by means of a highresolution six degrees of freedom motion detector. The three-dimensional shape of the first premolar was measured with following methods. A 3-D digitizer with a touch sensor probe was used to measure the coronal part of a cast stone model of the tooth. Computerized tomography data were used to obtain the root shape of the tooth. These movement and configuration data were combined to perform computer graphics analysis of the three-dimensional tooth movements. Hard acrylic dental appliances were fabricated for the upper and lower dentitions of each subject. Miniature strain-gauge transducers were mounted to the upper dental appliance at the right and left first molar regions. In addition, thin metal plates that contact the strain-gauge transducers were attached to the lower dental appliance. After a 1-week familiarization with the appliances, nocturnal bite force was measured for three nights at the home of each subject. From the 30 recordings, 499 bruxism events that met the definition criteria were selected. The above described system was also used to measure the maximum voluntary bite forces during the daytime. The mean amplitude of detected bruxism events was 22.5 kgf (s.d. 13.0 kgf) and the mean duration was 7.1 s (s.d. 5.3 s). The highest amplitude of nocturnal bite force in individual subjects was 42.3 kgf (15.6-81.2 kgf). Maximum voluntary bite force during the daytime was 79.0 kgf (51.8-99.7 kgf) and the mean ratio of nocturnal/daytime maximum bite force was 53.1% (17.3-111.6%). In this article, Peak Sound Pressure Level (PSPL) and Mean Poer Frequency (MPF) were studied, and the relation of both parameters was investigated.
