"三剤併用が奏効した一次性舌痛症の3例","Primary Glossodynia Well-Treated with Triple-Drug Combination: A Case Series","桃田 幸弘, 高野 栄之, 可児 耕一, 松本 文博, 青田 桂子, 山ノ井 朋子, 高瀬 奈緒, 宮本 由貴, 小野 信二, 東 雅之","Yukihiro Momota, Hideyuki Takano, Kohichi Kani, Fumihiro Matsumoto, Keiko Aota, Tomoko Yamanoi, 高瀬 奈緒, Yuki Miyamoto, Shinji Ono, Masayuki Azuma","null","一次性舌痛症は舌に痛みなどの異常感を訴えるものの,器質的または精神的要因のいずれも見出せないものとされる.近年,本症と神経障害性疼痛との関連が論じられている.今般,われわれはプレガバリン,トラマドール塩酸塩/アセトアミノフェン配合錠および加工附子末製剤の三剤併用が奏効した本症の3例を経験したので,その概要を報告する.患者は男性2名,女性1名,年齢42∼79歳(平均54.3歳)であった.全例に対してこれら三剤を併用した.痛みは緩解し,健康関連QOLも向上した.特記すべき有害事象は認められなかった.本症に対するこれら三剤併用の有用性が示唆された.","Primary glossodynia is characterized by abnormal sensations such as tongue pain without pathological changes and is not fully understood. Primary glossodynia is reported to be associated with neurophathic pain. We describe the cases of 3 patients(2 males, 1 female, mean age 54.3 years, range 42-79 years) with primary glossodynia that was treated with the concomitant medication of pregabalin (50-300mg/day), tramadol hydrochloride/acetaminophen tablets (37.5/325-150/1,300mg/day), and powdered processed aconite roots (0.5-1.5mg/day). This medication remarkably relieved the patients primary glossodynia and promoted health-reated quality of life of them. Significant adverse events did not occur in any of the cases.

Although CXCL10 and MMP-9 are both associated with pSS pathogenesis, the potential relationship between these two factors has not been investigated. In this study, we used LSG sections from pSS patients and human salivary gland cell lines to investigate the relationship between CXCL10 and MMP-9. Immunofluorescence analyses revealed that CXCL10 and MMP-9 were co-expressed in the LSG of pSS patients, particularly in expanded ductal cells. Furthermore, RT-qPCR analyses on human salivary gland ductal NS-SV-DC cells confirmed that CXCL10 expression was induced by interferon (IFN)-γ, whereas that of MMP9 was stimulated by IFN-α, tumor necrosis factor-α, and interleukin-1β. Remarkably, MMP-9 inhibition in IFN-γ-stimulated NS-SV-DC cells significantly decreased CXCL10 mRNA and secreted protein levels. Further analyses established that MMP-9 inhibition in IFN-γ-stimulated NS-SV-DC cells decreased STAT1 phosphorylation and hence suppressed IFN-γ signaling. Collectively, these results suggest that in addition to its reported role in the destruction of acinar structures, MMP-9 is involved in the IFN-γ-induced production of CXCL10 in pSS lesions. Here, we report a case of a patient with CyN who showed different periodicity and oscillations of neutrophil count compared with her mother, despite sharing the same novel genetic mutation. A 17-year-old Japanese girl who had been diagnosed with CyN shortly after birth presented to our hospital with a complaint of mobility of her teeth and gingivitis. Upon presentation, an intraoral examination was performed and revealed redness and swelling of the marginal and attached gingiva. Radiographs revealed extreme resorption of the alveolar bone and apical lesions in her mandibular lateral incisors. The patient's hematologic data demonstrated a lack of blood neutrophils (0/μL). The patient had no history of dental extraction, and her mother also had a history of CyN. The patient was diagnosed with severe periodontitis that was associated with CyN. Gene testing showed a novel heterozygous mutation in exon 4 of the ELANE gene (c.538delC, p.Leu180Ser fsX11). Based on the clinical findings, we planned to extract the patient's mandibular lateral incisors. Although the tooth extraction was scheduled considering the cyclic variation in neutrophil count, the patient's neutrophil count was 0/μL on the day before the planned extraction. Therefore, granulocyte-colony stimulating factor (G-CSF) was administered to increase the patient's neutrophil count. On the day of the patient's admission for the tooth extraction, she presented with fever (body temperature, 38.5°C), tonsillitis, and stomatitis. The extraction was subsequently delayed, and the patient was administered antibiotics and G-CSF for 4 days. At this time, the neutrophil count increased to 750/μL, and the tooth extraction was carried out safely. The postoperative course was uneventful, and the healing process at the extraction site was excellent. There is a possibility that the periodicity and oscillations of neutrophil count may change with growth in patients with CyN. Lingual nerve injury is a common complication following dental implant placement. CPA masses are likely to cause symptomatic trigeminal neuralgia, and thus can mimic and be easily confused with oral diseases. We experienced a case of CPA mass mimicking lingual nerve injury after dental implant placement. The patient was a 57-year-old Japanese female and complained of glossalgia. She underwent dental implant placement in the mandible before visiting our clinic. Panoramic x-ray radiography revealed no abnormalities; the salivary flow rate by gum test was 7.0 ml/10 min. She was diagnosed with lingual nerve injury and secondary burning mouth syndrome. Vitamin B12 and oral moisturizer did not provide relief; furthermore, numbness in the lower lip emerged. A Semmes Weinstein test demonstrated elevation of her sensitivity threshold. Finally, magnetic resonance imaging revealed a 20-mm-diameter mass in the CPA. The patient is now being followed under conservative management. Our experience underscores the importance of including CPA mass in the differential diagnosis of dental diseases. The Japanese version of the MOS 36-item short-form health survey version 2 (SF-36v2) is a questionnaire to measure health-related quality of life (HRQoL) in various diseases. We studied HRQoL in 26 primary glossodynia patients who self-completed the SF-36v2. The mean scores on the SF-36v2 subscales in our female patients were significantly lower than the national norms. Furthermore, those in the elderly female patients were significantly lower than those in the middle-aged ones. Conditions of primary glossodynia in the elderly patients were complicated by physical and mental factors; psychosocial supports should be required for their care in addition to pain control. In this study, we examined the preventive effect of γ tocotrienol on the 5 FU induced ROS production in human oral keratinocytes (RT7). We treated RT7 cells with 5 FU and γ tocotrienol at concentrations of 10 µg/ml and 10 nM, respectively. When cells were treated with 5 FU alone, significant growth inhibition was observed as compared to untreated cells. This inhibition was, in part, due to the ROS gene rated by 5 FU treatment, because N acetyl cysteine (NAC), a ROS scavenger, significantly ameliorated the growth of RT7 cells. γ tocotrienol showed no cytotoxic effect on the growth of RT7 cells. Simultaneous treatment of cells with these agents resulted in the significant recovery of cell growth, owing to the suppression of ROS generation by γ tocotrienol. Whereas 5 FU stimulated the expression of NF E2 related factor 2 (Nrf2) protein in the nucleus up to 12 h after treatment of RT7 cells, γ tocotrienol had no obvious effect on the expression of nuclear Nrf2 protein. Of note, the combined treatment with both agents stabilized the 5 FU induced nuclear Nrf2 protein expression until 24 h after treatment. In addition, expression of Nrf2 dependent antioxidant genes, such as heme oxygenase 1 (HO 1) and NAD(P)H:quinone oxidoreductase 1 (NQO 1), was significantly augmented by treatment of cells with both agents. The usefulness of perioperative oral function management aiming at the outbreak decrease in treatment complications and an early discharge was recognized. As a result, perioperative oral function management fee was founded at revision of medical service fees in Fiscal year 2012. In this clinical study, we evaluated the implementation of perioperative oral function management in Tokushima University Hospital. We examined 781 patients, including 563 patients for surgery and 218 patients for chemotherapy and radiotherapy. The mean age of patients was 58.8 ± 12.4 years old. The implementation rate of perioperative oral function management was 9.7% in the patients of surgery, and 17.4% in those of chemotherapy and radiotherapy. The highly required medical department was neurosurgery in the patients of surgery, and hematology in those of chemotherapy and radiotherapy. The mean number of tooth present was 21.3 ± 7.1 in the patients of surgery, and 19.8 ± 7.2 in those chemotherapy and radiotherapy. The rate of dental treatment was required in 40.5% of total patients who received surgery, and in 51.4% of patients who received chemotherapy and radiotherapy.The rate of patients who received denture treatment attained to 11.9% of the whole patients receiving surgery, and 13.3% of patients receiving chemotherapy and radiotherapy. It was revealed that there were many patients required potential demands in perioperative oral function management, and that there were many patients who need dental or denture treatment. OD-associated autonomic dysfunction is induced by postural changes and easily leads to disturbance in circulatory dynamics; however, most dental practices have not yet realized the importance of managing OD. We measured autonomic activity in a patient with OD during dental therapy and assessed the clinical significance of HRV analysis for OD. The patient was a 17-year-old Japanese female. She was diagnosed with impacted wisdom teeth and had no previous history of a distinct systemic disease. A surgical procedure to extract the teeth was safely performed under both local anesthesia and sedation with nitrous oxide and midazolam. After the surgery, her postural change to sitting induced orthostatic hypotension. HRV variables showed parasympathetic dominance due to the upright position. Subsequently, her posture was returned to supine, and atropine sulfate administration for the immediate treatment of OD returned her blood pressure to normal levels. HRV variables showed relative sympathetic dominance due to an atropine-derived parasympathetic blockade. A 60-year old woman who underwent dental implant placement by a dental practitioner visited our hospital because of postoperative mental hypoesthesia. Orthopantomography and CT revealed perforated bone defects at the posterior molar and ramus region of the mandible, but could not demonstrate a causal relationship between the implantation and the hypoesthesia. In blood test results, the values of sIL-2R and LDH were abnormally elevated. A tumor arising from soft tissue around the molar region was biopsied, and a diagnosis of diffuse large B-cell lymphoma was confirmed histopathologically. This tumor was treated at the Department of Hematology by the R-CHOP chemotherapy regimen, resulting in complete remission of the tumor, and masticatory function was adequately restored using dental implants. It is caused by the acquirement of factor VIII inhibitor. We report a case of acquired hemophilia A incidentally diagnosed on bleeding after tooth extraction. A 75-yearold woman referred to our hospital because of bleeding after tooth extraction. Hematological examination revealed a prolonged activated partial thromboplastin time (APTT) and decreased coagulation activity of factor VIII. In addition, a high level of factor VIII inhibitor was detected. Therefore, our patient was given a diagnosis of acquired hemophilia A. The patient was admitted to our hospital and received corticosteroids and bypass hemostasis using recombinant coagulation factor VII. On hospital day 42, APTT and factor VIII activity returned to the normal range. Moreover, factor VIII inhibitor disappeared. We previously demonstrated that tumor necrosis factor-α (TNF-α) induced matrix metalloproteinase-9 (MMP-9) production by the stimulating transcription factor, nuclear factor-κB (NF-κB), in human salivary gland acinar cells and that cepharanthin, a biscoclaurine alkaloid, prevented the activation of TNF-α-induced NF-κB activity. In addition, we have shown that cepharanthin prevented the destruction of acinar tissues in murine SS. Thus, in this study, we investigated the clinicopathological effect of cepharanthin on the patients with SS.
Ten patients with a diagnosis of primary SS were enrolled in this study. They were treated with cepharanthin for three months, followed by the evaluation of salivary production, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), anti-SS-A antibody, anti-SS-B antibody, pathological examination of labial salivary gland biopsy specimens, and the symptom of drymouth.
Average salivary production was 4.25ml/10minutes before the study. This average production increased to 6.46ml/10minutes after the study. There were no remarkable changes in CRP, ESR, anti-SS-A antibody and anti-SS-B antibody. In pathological findings, although the decreases in infiltration of lymphocytes were observed in 5 out of 5 cases, the reduction of destruction of acinar structures was clearly detected in 5 cases. Improvement of symptom of drymouth was detected in 7 out of 10 cases.

BMS is also known as glossodynia; both terms are used equivalently in Japan. Although the real cause of BMS is still unknown, it has been pointed out that BMS is related to some autonomic abnormality, and that stellate ganglion near-infrared irradiation (SGR) corrects the autonomic abnormality. Frequency analysis of heart rate variability (HRV) is expected to be useful for assessing autonomic abnormality. This study investigated whether frequency analysis of HRV could reveal autonomic abnormality associated with BMS, and whether autonomic changes were corrected after SGR. Eight subjects received SGR; the response to SGR was assessed by frequency analysis of HRV. No significant difference of autonomic activity concerning low-frequency (LF) norm, high-frequency (HF) norm, and low-frequency/high-frequency (LF/HF) was found between SGR effective and ineffective groups. BMS is also known as glossodynia; both terms are used equivalently in Japan. Although the real cause of BMS is still unknown, it has been pointed out that BMS is related to some autonomic abnormality, and that stellate ganglion near-infrared irradiation (SGR) corrects the autonomic abnormality. Frequency analysis of heart rate variability (HRV) is expected to be useful for assessing autonomic abnormality. This study investigated whether frequency analysis of HRV could reveal autonomic abnormality associated with BMS, and whether autonomic changes were corrected after SGR. Eight subjects received SGR; the response to SGR was assessed by frequency analysis of HRV. When cells were treated with either agent alone at a low dose, no significant cytotoxic effect was observed. However, the simultaneous treatment of cells with both agents almost completely suppressed cell growth. Whereas docetaxel stimulated the expression of nuclear factor-κB (NF-κB) p65 protein in B88 cells, γ-tocotrienol slightly inhibited the expression of constitutive nuclear p65 protein. Of note, the combined treatment with both agents inhibited docetaxel-induced nuclear p65 protein expression. "舌痛などの舌症状を主訴とする患者の臨床統計学的検討—舌痛症の特異性についてー.","Clinical Statistics of Patients with Lingual Abnormalities Including Glossalgia -Clinical Characteristics of Glossodynia-","桃田 幸弘, 高野 栄之, 可児 耕一, 松本 文博, 茂木 勝美, 青田 桂子, 山村 佳子, 大守 真由子, 東 雅之","Yukihiro Momota, Hideyuki Takano, Kohichi Kani, Fumihiro Matsumoto, Katsumi Motegi, Keiko Aota, Yoshiko Yamamura, 大守 真由子, Masayuki Azuma","null","目的:舌痛の器質的要因をスクリーニングする系統的診断システムを提案し,本法によって診断された舌痛関連疾患を検討する.方法:2007年1月から2009年12月までに舌痛などの舌症状を主訴としてで徳島大学病院歯科口腔外科を受診した患者104名(男性12名,女性92名,平均68歳2か月)を臨床統計学的に検討した.病歴聴取,口腔内・外診査,パノラマエックス線検査,血液検査,培養検査および唾液分泌検査を行った.局所麻酔薬と非ステロイド性抗炎症薬の効果も確認した.結果:原因疾患は口腔カンジダ症,口腔乾燥症,舌炎,舌痛症などであった.器質的変化や自覚症状の乏しい口腔カンジダ症や口腔乾燥症が認められた.舌痛症の多くは舌尖に発現し,食餌性刺激による誘発痛や圧痛が認められないものが多く,局所麻酔薬や非ステロイド系抗炎症薬の効果も乏しかった.舌痛症における味覚異常や低亜鉛血症の発現頻度は低く,手掌部発汗が高頻度に認められた.結論:培養検査や唾液分泌検査が不可欠であり,器質的変化や自覚症状の乏しい口腔カンジダ症や口腔乾燥症は舌痛症と診断される可能性があった.舌尖部疼痛を有し,食餌性刺激による誘発痛や圧痛がないことが舌痛症特異的な所見と考えられ,その病態に舌粘膜障害の関与は否定的であった.味覚障害や低亜鉛血症は舌痛症特異的な所見ではなかった.手掌部発汗は舌痛症特異的な所見と考えられ,その病態に自律神経異常の関与が示唆された.","Purpose: There are many kinds of diseases that can cause glossalgia, and it is not always easy to determine the differential diagnosis. Therefore, establishment of screening procedures for organic factors of glossalgia is important to obtain the correct diagnosis. We statistically analyzed the findings in patients with various lingual abnormalities. Methods: Patients were 104 persons consisted of 12 males and 92 females. Medical history taking, intraoral and extraoral examinarion, panoramic x-ray radiography, blood examination, culture, and salivation test were performed. It was also determined whether local analgesic or nonsteroidal anti-inflammatory drug relieved the pain or not. Results: Glossalgia-related diseases consisted of oral candidiasis, xerostomia, glossitis, glossodynia and other diseases. The major part of oral candidiasis lacked a change in organic appearance. Concerning xerostomia, a sense of thirst was also lacking. Local analgesic or nonsteroidal anti-inflammatory drug showed a limited effect on glossodynia. The prevalence of taste disorder or hypozincemia in glossodynia was less than that in other diseases. On the other hand the prevalence of palm sweating in glossodynia was greater than that in other diseases. Conclusions: Culture and salivation test were necessary for the diagnosis of either oral candidiasis without a change in organic appearance or xerostomia without a sense of thirst. If these examinations were not performed, these cases might be diagnosed as glossodynia. The presence of pain on tongue tip and the absence of food-induced pain or oppressive pain were specific findings of glossodynia. Lingual mucous damage could not be directly correlated with pathology of glossodynia judging from the nature of glossalgia and the result of analgesic effects. Taste disorder and hypozincemia were found to be nonspecific findings of glossodynia. 症例は76歳,男性.食事中,急に口蓋が腫脹し,呼吸困難を訴え救急搬送された.発作性心房細動,糖尿病,高血圧症でワルファリン療法中であった.正球性貧血とPT-INR値は1.52を呈した.呼吸・循環動態に異常なく,軟硬口蓋粘膜が広範に剥離していたが,圧迫止血後,粘膜弁を縫合閉鎖した.
症例は留意すべき口腔の出血性合併症であった.","We present a case of rapidly-enlarged and ruptured submucous hematoma of soft and hard palate during warfarin therapy.
The Patient was a 76-year old man. He suddenly had palatal swelling and difficulty in breathing while eating, and was brought to us by ambulance. He has been receiving warfarin therapy and treatments for paroxysmal atrial fibrillation, diabetes mellitus and high blood pressure. On blood examination normocytic anemia was diagnosed. Blood coagulation test demonstrated an elevated value (1.52) of PT-INR. The mucous membrane was exfoliated from both soft and hard palate without abnormalities in either respiratory or circulatory status. Immediately, suturing of the mucosa to the submucosal tissue was performed along with astriction.

The mechanisms underlying these reductions remain unclear. We have previously shown that TNF-α plays an important role in the destruction of acinar structures. Here we examined TNF-α's function in the expression of aquaporin (AQP) 5 in human salivary gland acinar cells. Immortalized human salivary gland acinar (NS-SV-AC) cells were treated with TNF-α, and then the expression levels of AQP5 mRNA and protein were analysed. In addition, the mechanisms underlying the reduction of AQP5 expression by TNF-α treatment were investigated. TNF-α-treatment of NS-SV-AC cells significantly suppressed the expression levels of AQP5 mRNA and protein, and reduced the net fluid secretion rate. We examined the expression and activation levels of DNA methyltransferases (Dnmts) in NS-SV-AC cells treated with TNF-α. However, no significant changes were observed in the expression or activation levels of Dnmt1, Dnmt3a or Dnmt3b. Although we also investigated the role of NF-κB activity in the TNF-α-induced suppression of AQP5 expression in NS-SV-AC cells, we detected similar TNF-α suppression of AQP5 expression in non-transfected cells and in a super-repressor form of IκBα cDNA-transfected cell clones. However, interestingly, chromatin immunoprecipitation analysis demonstrated a remarkable decrease in levels of acetylated histone H4 associated with the AQP5 gene promoter after treatment with TNF-α in NS-SV-AC cells. Global methylation levels in the salivary glands were significantly lower in the 5-Aza-CdR-treated mice than in the untreated mice. Moreover, the induction of demethylation in the AQP5 promoter of 5-Aza-CdR-treated mice was stronger than in the control mice. A 52-year-old man visited our hospital because of swelling and pain in buccal region on the right side in spite of chemotherapeutics at a practicing physician, whose diabetic nephropathy had been treated with dialysis treatment by the same physician. Orthopantomography elucidated an impacted wisdom tooth of the right side mandible and osteosclerosis around there. CT revealed inflammatory thickening of subcutaneous fat tissue from buccal to cervical region. In the blood examination, the values of WBCs, RBCs, platelets, CRP, AST, LDH, TG, BUN, creatinine, GLU, K and Ca increased abnormally, while those of hemoglobin, MCV,MCH,MCHC, albumin, Cl, decreased extraordinarily. Immediate medication of antibiotics was worked out under a clinical diagnosis of pericoronitis of lower wisdom tooth, periostitis of the mandible and cervical cellulitis. Nevertheless, rapid inflamation progressed downward; mediastinal abscess was found by CT image taken 6 days later from the beginning of the therapy. Immediately, extraction of pathogenic teeth, cervical drainage, and transcervical and transthoracic mediastinal drainage were performed under a diagnosis of descending necrotizing mediastinitis. Moreover, bacterial examination was performed using a pus specimen derived from abscess; α-Streptococcus and Prevotella intermedia were detected. CT taken 2 weeks later from the beginning of the therapy revealed continuing bilateral pleural and pericardial effusion in spite of intensive care. A 19-year-old man was referred to our hospital by a general practitioner bacause of discomfort of the tongue and difficulty in eating. The patient had a slight swelling of the tongue. Physical examination revealed a hard mass in the body of the tongue. Mandibular occlusal radiographs showed a radiopaque body measuring about 3×3mm in the center of the tongue. The clinical diagnosis was a foreign body in the tongue. The foreign body was removed surgically under local anesthesia. We performed element analysis of the removed foreign body. Therefore, it is necessary for apatite cement (AC) to be sterilized. Because there is little information on the sterilization methods of AC, the aims of this investigation were to evaluate the effects of various sterilization methods, including steam, dry heat, ethylene oxide (EtO) gas, and gamma irradiation sterilizations, on the setting and mechanical properties of AC. In the case of steam sterilization, because AC powder aggregated before setting-time measurements, the setting time could not be measured. When the powder was sterilized by dry heat or EtO gas, the setting time was prolonged significantly and the wet diametral tensile strength (DTS) value decreased significantly. Therefore, sterilizations with steam, dry heat, or EtO gas were suggested to be inappropriate methods for AC. Accordingly, the following experiments focused on gamma sterilization. The setting time of AC was retarded with an increase in gamma irradiation dose. The wet DTS value decreased with the increase in gamma irradiation dose. There was no compositional change due to the gamma irradiation. The following tests were carried out in order to examine the effect of the gamma irradiation on the setting reaction of AC in detail. Tetracalcium phosphate [TTCP: Ca(4)(PO(4))(2)O] and dicalcium phosphate anhydrous (DCPA: CaHPO(4)) were separately irradiated, and the cements were produced with the use of irradiated powder and nonirradiated powder. Although the wet DTS value of AC produced from irradiated TTCP and nonirradiated DCPA decreased with increasing gamma irradiation dose, there was no significant difference. In contrast, the wet DTS value of AC produced from irradiated DCPA and nonirradiated TTCP significantly decreased with the increase in gamma irradiation dose. Although the setting reaction of HAP putty and resulting transformation to HAP is the reason for this excellent biocompatibility, it also limits the handling period. In this study, the relationship between the setting reaction of HAP putty and its hemostatic ability was investigated, where neutral sodium hydrogen phosphate concentration and post-preparation time were used as indexes. A higher concentration of neutral sodium hydrogen phosphate was found to result in decreased hemostatic ability. The hemostatic ability of HAP putty decreased with time after its preparation; thus it was important to use HAP putty within 10 min after its preparation. Although the setting reaction of HAP putty and resulting transformation to HAP is the reason for this excellent biocompatibility, it also limits the handling period. In this study, the relationship between the setting reaction of HAP putty and its hemostatic ability was investigated, where neutral sodium hydrogen phosphate concentration and post-preparation time were used as indexes. A higher concentration of neutral sodium hydrogen phosphate was found to result in decreased hemostatic ability. The hemostatic ability of HAP putty decreased with time after its preparation; thus it was important to use HAP putty within 10 min after its preparation. Fifteen patients who had received implants (Brånemark system, Nobel Biocare, Gotebörg, Sweden) in the edentulous mandible and completed a 1-year follow-up after the fitting of implant-anchored fixed prostheses were selected. In seven patients (Group A), four or five implants were installed between the mental foramina, and in eight patients (Group P), one or two implants, one on each side, were installed in the posterior regions in addition to the implants between the foramina. All implants of both groups achieved osseointegration. In Group A, there was no implant loss after loading. Six implants were lost in five patients of Group P within 1 year after loading. All of them were located in the posterior region. To elucidate whether or not the failure rate of the implants in the posterior region of Group P after loading was especially high, the failures were also compared with 89 implants, which were installed in the posterior region of the mandibles to support implant-anchored fixed partial prosthesis, during the same period (Group C). The cumulative survival rate of the implants of Group P was 60%, while that of the implants of Group C was 100% (P<0.001). When the survival rates of posterior implants with the same length of the two groups were compared, there were significant differences for the 7- and 10-mm-length implants only. These data demonstrate that the posterior implants in Group P are at greater risk. Deformation of the mandible due to jaw movement was thought to be the most likely cause of the implant loss. The setting times were measured for aw-AC(chi) containing various amounts of flomoxef sodium. X-ray diffraction (XRD) analysis was also conducted for the identification of products. To evaluate the drug release profile, set aw-AC was immersed in saline and the released flomoxef sodium was determined at regular intervals. The setting time was prolonged slightly with the addition of flomoxef sodium. The difference at 10% flomoxef sodium (0% vs. 10%) was not significant (p>0.05), and can be negligible in clinic. The XRD analysis revealed that formation of hydroxyapatite (HAP) from aw-AC(chi) was reduced, even after 24 h, when the aw-AC(chi) contained flomoxef sodium at 8% or more. The flomoxef sodium release from aw-AC(chi) showed the typical profile observed in skeleton type drug delivery system (DDS). Changing the concentration of chitosan can control the rate of drug release from aw-AC. Histopathological examination revealed Aspergilli in the extirpated tissue of the maxillary sinus. Serological examinations showed elevated serum levels of galactomannan and β-D-glucan. Galactomannan is a specific marker for Aspergilli, and β-D-glucan is specific for fungi.
