Schwannomas are benign, slow-growing tumors comprising well-differentiated Schwann cells that can arise along the course of nerves in the body. Intrapulmonary schwannoma represents an extremely rare form of neoplasms. Here, we report the case of a 70-year-old man with intrapulmonary schwannoma coexisting with rectal carcinoma and describe related results obtained using computed tomography (CT), magnetic resonance imaging (MRI), and 18-F-fluorodeoxyglucose (18F-FDG) positron emission tomography/CT. This case indicates that intrapulmonary schwannoma could appear as a slightly heterogeneous hypodense tumor on CT and as a pulmonary nodule with high signal intensity on T1-weighted images. Although ruling out the possibility of pulmonary metastasis from rectal carcinoma before transbronchial lung biopsy is difficult, the diffusion-weighted image and low 18F-FDG accumulation could provide a clue to diagnose intrapulmonary schwannomas.
