(Summary) AsisTIVA is total intravenous anesthesia support software for syringe pump control. It remains the only automated program to have achieved both practical and commercial viability. AsisTIVA has been certified for use at Tokushima University, Tohoku University, and at its developer institution, Fukui University. We conducted our initial anesthesia management with AsisTIVA at Tokushima University. This initial experience was successful, with no adverse effects. AsisTIVA controls propofol and remifentanil concentrations to maintain a Bispectral index of 45 and adjusts rocuronium concentrations to maintain a Train Of Four value of 1. By managing these anesthetics, AsisTIVA allows anesthesiologists to focus on other aspects of anesthesia management. Its use is expected to expand further. J. Med. Rikako Takahashi, Ryosuke Kawanishi, Nami Kakuta, Hiroki Yonezawa, Hiroya Endo and Katsuya Tanaka : Severe bradycardia with cardiac arrest caused by test stimulation during the second vagus nerve stimulator implantation: A case report, Circulation Control, 45, 1, 39-42, 2024. (Summary) Vagus nerve stimulation (VNS) is used to treat drug-resistant epilepsy. We report a case of intraoperative severe bradycardia and cardiac arrest during a second VNS implantation. No cases of bradycardia during the second VNS implantation have been reported. <br> The patient underwent the first VNS implantation before and experienced no intraoperative bradycardia. A mechanical error of the VNS was discovered, and the second operation was scheduled. General anesthesia was scheduled with supporting by a prophylactic pacing defibrillator to manage bradycardia during VNS. After exchanging the electrodes on the left vagus nerve, test stimulations were performed. We observed an increase in the PR interval on the electrocardiogram (ECG) during the test stimulation and severe bradycardia with cardiac arrest during the main stimulation, which required shortterm chest compressions and percutaneous pacing. After discontinuing stimulation, the pulse rate recovered immediately. We conclude that continuous ECG monitoring intraoperative is crucial, even if the first VNS implantation was uneventful. Hiroki Yonezawa, Ryosuke Kawanishi, Hideto Sasaki, Yohei Sogabe, Kentaro Hirota, Yasuto Honda and Katsuya Tanaka : Insertion of a fogarty catheter through a slip joint section for neonatal and infantile one-lung ventilation: A report of two cases, The Journal of Medical Investigation : JMI, 68, 1.2, 209-212, 2021. (Summary) Here, we report two cases involving a neonate and child in which a slip joint section was used to thread a Fogarty catheter into the endotracheal tube for one-lung ventilation (OLV). Both the neonate and infant required OLV, and were placed under general anesthesia. A Fogarty catheter was used for OLV. The Fogarty catheter was passed into the intraluminal side of the endotracheal tube through a slip joint section. OLV was maintained successfully without severe air leakage or Fogarty catheter displacement. The neonate had been intubated pre-operatively with a 3.5-mm inner diameter endotracheal tube, and we used that tube. These cases indicate that the technique can be applied to pre-operatively intubated patients and does not require surgeons to exchange endotracheal tubes.
